POLICIES

Fees

  • With Insurance: You are responsible for a co-pay or deductible, depending on your insurance.

  • Without Insurance: An evaluation appointment is $300 and a follow up appointment is $150.

Payments

I accept insurance or cash, check and credit cards. I expect payments at the end of session, unless we have agreed on other arrangements.

Cancellations

If you must cancel or reschedule an appointment, I require 24 hour notice (weekends not included). If your appointment is on a Monday, the cancellation must be made by the same hour on the preceding Friday. Cancellations that occur with less than 24 hour notice or failure to show for an appointment will be charged the full amount. An evaluation appointment is $300 and a follow up appointment is $150.

No-Shows

Arrival more than fifteen minutes after the start of the appointment will be considered a missed appointment and the cancellation fee will apply.

Medication Refills

Refill requests must come directly from clients (i.e. requests from pharmacies will not be answered). Medications will be refilled during normal business hours and only after a follow-up appointment is scheduled. Refill requests on weekends will be filled the following Monday or non-holiday business day.

Contacting me

I make every effort to return calls or text within the same day. When you leave a voicemail please include your name, reason for call, and your telephone number. Messages received late in the day, on weekends, or holidays may not be returned until the next business day. If you are experiencing a medical or psychiatric emergency please call 911 or proceed to the nearest emergency room.
